Using Telehealth for Chronic Care Management and Coordination

Millions of people live with at least one chronic condition, making their health a round-the-clock, seven-days-a-week exercise in care management and coordination. Telehealth can make that process easier by giving patients and caregivers online platforms and digital health devices to share data and care plans. The challenge lies in finding the right methods and technologies to engage the patient and give the provider the information he or she needs to improve management and outcomes.

As Vice President of Connected Health for Partners HealthCare, Dr. Joseph Kvedar has been at the forefront of the digital health movement for many years. Through his leadership at Partners’ Center for Connected Health, he’s helped the Massachusetts-based health system research and develop many new platforms and tools for connected care. He had helped Partners become a national leader in using new technology to improve the delivery of health.

In this presentation Dr. Kvedar will share his experiences with telehealth and digital health and explain how Partners is propelling chronic care management and coordination into a new era.
